Supplemental Final Payroll forms are to be submitted at the time of final certified payroll submittal.

Please ensure that the following forms are attached before clicking the Submit button below. Blank copies and templates of these forms can be downloaded. If a form is omitted (for example, if it is not applicable to the current project), please note such in the comment box below. Attachments must be in PDF format with relevant file names.

  • *S3R-1 - Section 3 Notice Documentation (all items will likely be non-applicable).
  • *S3R-2 - Business Utilization Report. Fill out what is applicable. (Subcontractors, only if you hire subcontractors or lower tier contractors.)
  • *S3R-3 - Applicant and New Hire Documentation. All items apply; fill out completely.
  • **Yearly EEO Report

* Applicable for contract bids exceeding $100,000 

** Applicable if contractor(s) has/have more than 50 employees with a covered contract or subcontract of $50,000 or more.
